Send a useful support request
To help us investigate efficiently, include:
- your clinic name and the email address associated with your account;
- the screen or workflow you were using and the result you expected;
- what happened instead, including the exact error message and approximate time; and
- a screenshot with unnecessary personal or sensitive information removed.

Patients should normally contact the clinic or practice that created the record. That organization controls the record and is best positioned to verify identity, authority, and applicable retention requirements.
Urgent and emergency situations
Docuchart support is not an emergency service
For a suspected account compromise or security issue, email support@docuchart.com promptly with “urgent security issue” in the subject line. Do not include passwords or sensitive records in the initial email.
