Privacy Choices

Use this page to understand and exercise privacy choices related to Docuchart.

Choose a request

Privacy rights vary by location and by whether Docuchart or one of our customer organizations controls the information. We will evaluate a verified request under the laws and agreements that apply.

You may also unsubscribe from optional marketing emails using the link in the message and manage cookies or local storage through your browser settings. Disabling essential storage may prevent sign-in or other core functions.

Customer-controlled records

Most information entered into Docuchart belongs to and is controlled by the organization using the Services. That organization decides why the information is collected, who can access it, and how long it must be kept.

Patients should usually contact their clinic first

If your request concerns a patient chart, consent, treatment photo, face chart, clinical note, invoice, or other clinic record, contact the clinic or practice that provided your care. The clinic controls that record and can verify your identity, authorized representative, and applicable medical-record retention duties. We will assist the clinic with a verified request where required.

How to submit a request

Email support@docuchart.com from the address associated with your account when possible. Use a clear subject such as “Privacy request: access” or select one of the request cards above.

Include only what is needed to locate and understand the request:

  • your name and account email;
  • the customer organization involved, if any;
  • the type of request and the information or date range involved; and
  • whether you are making the request for yourself, an organization, or another person.

Do not email passwords, full payment-card details, government identification, medical records, or treatment photos unless we provide a secure method and specifically ask for them.

Identity and authority verification

Before acting on a request, we may confirm control of an account or email address and ask for information reasonably necessary to match the request to our records. A request made for another person or an organization may require proof of authority. We use verification information only to evaluate and complete the request and protect against unauthorized disclosure or deletion.

If we cannot verify a request, do not control the information, or need the customer organization to decide it, we will explain the next appropriate step when permitted. We respond within the period required by applicable law.

Deletion, retention, and request limits

A deletion request does not always require immediate removal of every copy. Information may be retained where reasonably necessary to comply with legal or professional recordkeeping, complete transactions, provide security and audit history, prevent fraud, resolve disputes, enforce agreements, or preserve another person's rights. Limited copies may also remain in protected backups until overwritten through standard cycles.

We may deny or limit requests that are fraudulent, abusive, unreasonably repetitive, technically infeasible, would expose another person's information, or are not required by applicable law. Where required, we will explain the basis and available appeal process.
